CCC Plus Waiver란 무엇인가요?
Commonwealth Coordinated Care Plus (CCC Plus) Waiver는 Virginia Medicaid의 Home and Community-Based Services(HCBS) Waiver 프로그램입니다.
요양시설(Nursing Facility) 또는 기타 전문 의료시설 수준의 돌봄이 필요할 수 있는 사람이 자격 기준을 충족하는 경우, 가정과 지역사회에서 필요한 장기 서비스와 지원을 받을 수 있도록 돕습니다.
CCC Plus Waiver는 모든 연령(All Ages)을 대상으로 하며, 개인의 필요와 자격 기준에 따라 서비스가 제공됩니다.
누가 신청할 수 있나요?
CCC Plus Waiver는 의료적·기능적 필요가 있어 장기 서비스와 지원(Long-Term Services & Supports, LTSS)이 필요한 사람을 위한 프로그램입니다.
Virginia의 LTSS Screening에서는 다음과 같은 사항을 평가합니다.
• Activities of Daily Living (ADLs)
일상생활 활동에 어느 정도의 도움이 필요한지
• Physical Health Status
신체 및 건강 상태
• Medical or Nursing Needs
지속적인 의료 또는 간호 지원이 필요한지
• Risk of Institutional Care
필요한 서비스와 지원이 제공되지 않을 경우 Nursing Facility 입소 또는 입원의 위험이 있는지
최종 자격 여부는 개인의 상황에 대한 공식 Screening 및 Medicaid 자격 심사를 통해 결정됩니다.
어디서부터 시작하면 될까요?
CCC Plus Waiver를 알아보고 있다면 거주 지역의 Department of Social Services (DSS)에 연락하여 Long-Term Services & Supports (LTSS) Screening을 요청하는 것이 첫 단계입니다.
Community-Based Screening Team이 신청자의 건강 상태와 일상생활 지원 필요 등을 평가합니다. Screening Team에는 지역 Department of Social Services 및 Health Department 관계자가 참여할 수 있습니다.
병원에 입원 중인 사람이 Screening이 필요한 경우에는 병원의 Discharge Planner를 통해 진행할 수도 있습니다.
신청 절차
STEP 1 — Request an LTSS Screening
LTSS Screening을 요청합니다.
거주 지역의 Department of Social Services에 연락하여 CCC Plus Waiver를 위한 LTSS Screening에 대해 문의합니다.
STEP 2 — Complete the Screening
필요한 Screening을 진행합니다.
Community-Based Screening Team이 신청자의 일상생활 지원 필요, 건강 상태, 의료 및 간호 필요 등을 평가합니다.
STEP 3 — Apply for Medicaid
Medicaid 자격 신청을 진행합니다.
CCC Plus Waiver 서비스를 Medicaid에서 지원받으려면 Medicaid 자격 요건도 충족해야 합니다.
Medicaid 신청은 Virginia의 온라인 신청 시스템을 이용하거나, Cover Virginia에 전화하거나, 거주 지역 Department of Social Services를 통해 신청할 수 있습니다.
STEP 4 — Waiver Eligibility & Services
자격 승인 후 필요한 서비스를 선택합니다.
Medicaid 자격 및 CCC Plus Waiver Screening 요건을 충족하면 개인의 필요에 따라 이용 가능한 Waiver 서비스를 선택하게 됩니다. Screening Team은 해당 지역의 서비스 제공자를 찾는 데 도움을 줄 수 있습니다.
