Virginia Guardian Reporting Requirements – Form CC-1644
버지니아에서 법원에 의해 성인 후견인(Guardian)으로 지정된 경우, Report of Guardian for an Incapacitated Person (Form CC-1644)을 정해진 기간에 제출해야 합니다.
후견인은 피후견인(Incapacitated Adult)의 생활 상태, 건강 및 복지 등에 관한 정보를 정기적으로 보고해야 하며, 제출 전 Virginia Courts에서 최신 CC-1644 양식과 작성 지침을 확인하시기 바랍니다.
보고 시기
Initial Report (최초 보고): 후견인 자격 취득 후 최초 4개월에 대한 보고
Annual Report (연례 보고): 이후 매 12개월마다 제출
Final Report (최종 보고): 피후견인의 사망, 후견인 변경, 다른 주로의 후견 이전, 권리 회복 등 후견 관계가 변경되거나 종료되는 경우 제출
Fairfax County 제출 안내
해당 보고기간 동안 피후견인(Incapacitated Adult)이 마지막으로 거주한 지역이 Fairfax County인 경우, 작성한 보고서를 Fairfax County Department of Family Services의 Adult Protective Services/Guardianship Unit으로 제출합니다.
현재 Fairfax County 안내에 따르면 보고서 제출 시 $5 filing fee가 있으며, 수표 또는 Money Order는 “Fairfax County” 앞으로 작성합니다.
